General Office Policies

 

Basic Policy

Payment for service is due in full at the time the service is provided in our office.

 

For Patients with Insurance

All major forms of insurance are accepted. We will collect any Co-payments on the day of your appointment if the deductible has been met and bill the insurance for the remainder. If you have not met your deductible or you do not have insurance, payment in full will be asked for at the time of service, (Visa, MasterCard cards are accepted.) It is the responsibility of the patient to make sure all insurance information is correct and up to date ( a copy of the insurance card is required.

 

Medicare Patients

We will bill Medicare for you. We will also bill secondary insurances for you. All co-payments or deductibles are due and payable at the time service is provided. A referral from your primary care physician is required.

 

Medicaid  Patients

All Medicaid  patients must provide a current, valid sticker or card before being seen. A referral from your primary care physician is required.

 

Surgery Fees

A deposit is required prior to most surgeries.

All co-pays, deductibles are due prior to your surgery. Our office will obtain any prior authorization by your insurance carrier.

 

No-Covered Services

Any care not paid for by your existing insurance coverage will require payment in full at the time services are provided or upon notice of insurance claim denial.

 

Personal Injury Cases

This office does bill for auto accident or liability or lawsuit-related cases. You are responsible for payment at the time of service. We do not accept liens.

 

Your Health Checks
